Statement of Information Practices
The following statement describes how medical information about patients may be used and disclosed and how patients can get access to this information

Preterm is committed, and required by law, to maintain the privacy of patient personal health information (“PHI”), including maintenance of reasonable and appropriate physical, administrative, and technical safeguards to protect that information, and to provide patients with notice of our legal duties and privacy practices with respect to PHI

Per the HIPAA Privacy Rule, PHI cannot be used in pursuit of criminal, civil, or administrative investigations seeking prosecution related to any reproductive health care a patient receives, including abortion, that is lawful under the Constitution and laws of the State of Ohio

Preterm is permitted to use protected information only for the purposes of:

Treatment: for example, we may provide information to a subsequent provider for treatment in the case of a medical emergency

Payment: for example, we may provide identifying information to patient's insurance company for billing purposes.

Healthcare Operations: for example, we may use patient information for internal quality assurance assessment to evaluate our quality of medical care

Any other uses or disclosures will be made only with patient written authorization

Preterm may contact patients to provide appointment information or follow-up care

Patients have the right to request restrictions on certain uses and disclosures of information, although Preterm is not required to agree to the restriction

Patients have the right to receive confidential communication of their information

Patients have the right to inspect and obtain a copy of their information. Preterm reserves the right to charge a reasonable, cost-based fee for making copies

Patients have the right to amend their information. Preterm requires a written request, including the reason for amendment, and has the right to deny the request

Patients have the right to receive an accounting of any disclosures of their information

Patients have the right to obtain a copy of this notice upon request
